  • Appointment details: requested service, preferred time, and brief reason for visit (please do not include detailed health information through our website).
  • Payment information: processed by Square; we never store full card numbers on our systems.
  • Website usage: IP address, browser type, device, pages viewed, and referring URLs.

Protected Health Information collected during a clinical encounter is handled in accordance with HIPAA and our separate HIPAA Notice of Privacy Practices. Our website contact form and general email are not secure channels and should not be used to transmit PHI.
